New Client Information/Parent Intake

The parent intake appointment serves as a preliminary evaluation. During the parent intake the goals and concerns of the parents are identified, then a placement will be made according to the decisions of the clinic director.


Step 1: Interest in services at Little Spirits Bay Area Inc. is expressed through an email to: [email protected]


Step 2: An appointment is made and a $50 deposit will be required to hold your parent intake appointment. Credit card information will be taken and a $50 hold will be placed on the card.  The card will only be charged if you do not show up for the appointment and have not cancelled the appointment 12 hours in advance.  


Step 3: After the appointment is made, the office will email you the registration and intake forms to be filled out and emailed to [email protected], brought to your parent intake appointment or submitted before services begin. 


Step 4: During the parent intake, the clinic director and parents will discuss specific needs, concerns, scheduling, etc. and an appointment time will be made to start the initial assessment.
